Corporate/LLC Tenants
Thank you for your interest in a CBM property. Please provide us with the following:
- A completed CBM Leasing Application (see below)
- Copies of your company's current financial statements (preferably audited)
- Copies of your company's Articles of Incorporation or Articles of Organization, as applicable
- A copy of your company's by-laws
- A resolution authorizing the proposed transaction (indicating appropriate signers)
- Copies of your company's bank statements for the past two months
- Copies of your IRS tax returns (all schedules included) for the past two years
- A Certificate of Insurance naming the landlord as additional insured's

Individual Tenants
Thank you for your interest in a CBM property. Please provide us with the following:
- A completed CBM Leasing Application (see below)
- Copies of your company's bank statements for the past two months
- Copies of your company's IRS tax returns (all schedules included) for the past two years
- A copy of the grant deed for any real estate owned
- A copy of the year-ending mortgage statement for real estate owned
- A Certificate of Insurance naming the landlord as additional insured's

CBM Leasing Application
You can download our Leasing Application by clicking the above "Download CBM Leasing Application" button, or you can contact us and request we mail a copy to you.
When you've completed and signed your application, mail it to Centers Business Management, Attn: Leasing Application, 1517 S. Sepulveda Boulevard, Los Angeles, CA 90025, or fax it to 310-575-1147.
About Our Downloadable Leasing Application
Our Leasing Application is in PDF format. You will need Adobe Acrobat Reader, a free download, to open and print the file. Visit www.adobe.com to download the free Reader.
Please note if you are unfamiliar with using Adobe Acrobat Reader...
Adobe Acrobat Reader will let you to open and print our Leasing Application (or any other PDF file) and save a blank copy to your hard drive. However, Reader alone will NOT let you save the Leasing Application inclusive of information you type into its form fields.
- If you want to complete the Application on-screen, you will need to print out whatever pages you complete before you close the file, because when you reopen the file any information you typed in will no longer be there.
- That doesn't mean you have to complete the entire application in one sitting: Simply print out any pages you have completed before you close the file; when you reopen the file, begin with the page at which you left off, printing that and any other pages you complete in that sitting before you close the file again. When you put together the printouts from the various sittings you'll have a completed Leasing Application to forward to CBM.

CBM does not sell, share, or distribute information it collects through its Leasing Application. The Personal, Financial, and Questionnaire information you submit will only be shown to parties directly involved in reviewing your application, verifying information in it, and executing your lease.
